Perx Health digital app for managing chronic conditions actively engages the highest-risk, hardest-to-reach patients 4-5x per day. With proven ability to reach members with complex social determinants of health (SDoH), Perx Health has delivered on the promises of digital health in an unparalleled manner. Through systematic ‘precision’ behavioral change Perx Health personalizes condition management with a variety of extrinsic and intrinsic motivators (gamification, extrinsic rewards, challenges, community support, education) to engage individuals in a way that taps into their personal motivators. Participants in Perx programs have daily engagement with the app (72% of monthly activity) exceeding engagement of mobile consumer applications like Facebook and Twitter (66% and 46% respectively). The patient experience, via smartphone app, uses behavior science and AI-powered algorithms to drive behavior change, offering full treatment plan adherence and evidence-based methods to capture real-time data. For clinicians, Perx Care Portal is a platform where care teams can remotely prescribe and monitor treatment, perform telehealth consultations and communicate directly with patients through digital channels. In peer reviewed research by The University of Technology Sydney, Perx sustained long-term treatment adherence of 96% over 6 months. In a separate 12-month randomized controlled trial recently published in BMJ, patients managing complex comorbidities were randomly assigned the Perx smartphone App (vs standard of care). Those enrolled with Perx demonstrated statistically significant clinical and bio-marker improvement in a public health cohort of older demographic (average age 65) and lower-socioeconomic status.  Access is no issue with Perx Health, as the platform can live in ‘offline mode’ for those in rural communities. The Perx team is constantly updating the app to be as accessible as possible with “backwards” updates to cover those with older phones and less access to care. The Perx android app supports versions from Android 5 which covers 94% of android devices in the world. For iPhone users, Perx supports members with iOS 13+, covering 93% of users worldwide. Coverage is suspected to be even higher in developed countries such as the United States and Australia. After pioneering patient engagement in the Australian market Perx Health has expanded into the US across all 50 states &amp; territories. By the end of 2021 Perx will be available to 9M+ lives.
