Sarah Chouinard, MD is the Chief Medical Officer of Main Street Heath. For the last 15 years, Dr. Chouinard was the Chief Medical Officer of Community Care of West Virginia. Sarah also served as National Faculty for quality improvement initiatives at the Center for Medicaid and Medicare Service. She also served as Medical Director for West Virginia’s Public Employees Insurance Agency (PEIA) where she worked on value-based care initiatives. She was recognized by the West Virginia Rural Health Association as “Rural Practitioner of the Year” in 2008 and was featured in stories by Governing magazine, The N
